Please review this checklist provided by ApplyChineseVisa.com:
Tourist Visa (L Visa)
1. One recent passport photo. You can take passport photo at Costco, CVS, Wal-Mart, etc.
2. Valid and Actual Passport:
- at least 6 months of remaining validity
- at least one blank visa page
- Renewing passport can take very long time. If possible, try to use the Express Service.
- Make a copy of the first page of your passport (page with photo) and include it in your application package.
3. Complete Q1 form. A sample is here
- Affix the passport photo on the Q1 form.
- Chinese descendents are required to provide their Chinese name. It only helps.
- Provide explanation on Section 5 if applying for 2-year multiple entry.
- Provide detailed contact information both local and while in China.
- There should be no BLANK left in the application. ex. the kid's school name and address MUST BE written down in the form
If there is no information available, for example email address, need to write down "N/A" in the form.
- Sign & Date.
4. Fee Your application will not be processed until full fund in place.
- China Embassy fee is $140 per applicant for American citizen.
- cVille processing fee: $20 per applicant.
- Return shipping fee: $20 per package.
- Check Amount = total of the above three
- All fee should be in one check. We accept personal checks. Write check payable to cVille Visa Service
- For example, if all applicants are American citizens:

- If applying 2ME, write additional checks. Write a separate $20 check for each 2ME applicant. If you 2ME is rejected, this check will be returned to you.
5. If born in China, and it is the first time applying, please provide a copy of the first page of the Chinese passport.
6. If this is a kid of first time applying, provide a copy of one parent's passport/greencard and copy of Birth certificate(no need for originals)
7. If your passport is newly renewed and you've been to China before, provide a copy of your old Chinese visa.
8. Non-US citizens must submit a copy of their green card or valid US visa in addition to their original passport.

10. Children 2-year Multiple Entry Special Requirement:
For those children who were born in US and their parents are Chinese, if they want to apply for a 2-yr multiple entry visa, we need the following document:
1) a copy of birth certificate (dose not matter whether they have a visa before or not)
2) a copy of one of parent's passport showing they were born in China.
Business (F) Visa
Besides above items, the following are required:
- Copy of the Visa Notification Form, sealed by the appropriate Municipal Bureau of Commerce in China. The form can be obtained by the company in China.
- An invitation letter from the host company or unit in China and a business letter from the company in US explaining the purpose of the trip.
- For 2-year Multiple Entry Visa, need a detailed letter explaining precisely why explain precisely why the applicant needs to travel to China so often.

Study (X) Visa
Besides above items, the following are required:
- An approved Foreign Student Visa Application Form (JW201 or JW202) issued by the authorized Chinese unit, and an enrollment letter from the Chinese school.
- The applicant who will study in China for more than six months shall also provide a Physical Examination Certificate for Foreign Citizen.
